What is the Balke treadmill test and what is it used for?
The Balke treadmill test is a graded walking test in which treadmill speed remains constant while the incline progressively increases. Total exercise duration is associated with aerobic capacity and can therefore be used in regression equations to estimate VO₂max. The test is primarily useful for estimating cardiorespiratory fitness.
How does the Balke formula calculate VO₂max?
The calculator uses sex-specific regression equations developed for Balke treadmill testing. For men, the equation is VO₂max = 1.444 × T + 14.99, while the female equation is VO₂max = 1.38 × T + 5.22, where T represents total test time in decimal minutes. Seconds are therefore converted into a fraction of a minute before the equation is applied.

What is the MET value and how is it calculated?
MET is a standardized unit commonly used to express the intensity or energy cost of physical activity. The calculator derives the MET value by dividing estimated VO₂max by 3.5. For example, an estimated VO₂max of 35 ml/kg/min corresponds to approximately 10 METs.
How accurate is the Balke VO₂max calculator?
The accuracy of the estimate depends partly on how closely the treadmill test follows the protocol associated with the regression equation. Several Balke protocol variations exist, including differences in treadmill speed, grade progression and timing. A result from a substantially different treadmill protocol should therefore not automatically be interpreted using the same equation.
